Getting Started

Let's begin with the whole picture.

The form below is how we start. It turns the first meeting from an interview into a conversation.

Fill in what you can. Every question is optional except a way to reach you, and there is no wrong answer anywhere on this page. Whatever you leave blank, we can talk through together.

Initial meeting — family survey

Some sections are for the student and some for parents. It is fine to sit down and do it together, and equally fine to split it up.

It takes about fifteen minutes, and is best done in one sitting.

01Student information
02Academic profile

Approximate is fine — we'll go through the details together.

List exam and score.

03Standardized testing
Testing status

English / Math / Reading / Science.

04Activities, athletics & interests

Include any leadership roles.

Level: JV, varsity, club, intramural, or none.

05College preferences

Tick as many as apply. Nothing here is binding — it's a starting point.

School size
Geographic preference
Campus setting
Institutional type
06Academic interests & possible major
Certainty about major
Academic environment preference
07Financial considerations

Share only what you're comfortable sharing. It helps shape a realistic list, and nothing here affects whether we can work together.

Financial aid need

Only if you're comfortable sharing.

In-state vs out-of-state
08Student wellness & support needs

Everything in this section is optional, and treated in confidence.

Learning style
Social environment
09Family perspective

And any impressions worth passing on.

The family's role in the process
Stress level about the process
10In the student's own words

These are for the student to answer. There are no wrong answers, and short is fine.

Everything you share is held in confidence and used solely to guide your college planning. Nothing is stored on this website — the form is sent straight to clearingcbc@gmail.com and nowhere else.
